Active surveillance for low-grade prostate cancer: is it safe?
The authors reviewed the data regarding the outcomes of men treated with active surveillance for low-risk prostate cancer to determine if there was a subset of patients at an increased risk of progression compared with men who were treated with radical prostatectomy or radiation therapy. This retrospective analysis identified 18 patients treated with active surveillance. Mean age was 65.5 years (range, 55 to 74 years). The median time from diagnosis to initiation of active surveillance was 44 months (range, 18 to 159 months). The median initial serum prostate-specific antigen level was 3.1 ng/mL (range, 0.6 to 7.0 ng/mL) and median Gleason score was 6 (range, 5 to 7). The median follow-up from the time of initiation of active surveillance was 37.7 months (range, 0.6 to 132.1 months). The incidence of progression during active surveillance was 33% (6 of 18 patients). No patient was diagnosed with prostate cancer after active surveillance. One patient had an undetectable serum prostate-specific antigen level but had a prostate biopsy positive for Gleason grade 5 prostate cancer. Of the remaining 5 patients with rising serum prostate-specific antigen level, 4 of these 5 patients had a diagnosis of high-risk disease on biopsy. Active surveillance may be considered a viable alternative for low-risk prostate cancer. The risk of progression during active surveillance for low-risk prostate cancer is 33%. A subset of patients diagnosed with low-risk prostate cancer is at an increased risk of progression to high-risk disease, which may preclude treatment with active surveillance.Q:
